The origin of the Hospital of San Juan de Dios in Cali dates back to 1745, when the city council of this small town of 5,000 inhabitants welcomed the initiative of Don Juan Francisco Carreno, who, authorized by the Bishop of Popayán, Don Francisco Figueredo y Victoria, presented a plan to found a charity hospital. The proponents were the city council members, Mr. Francisco Domínguez de Riascos and Mr. Francisco Sanjurgo Montenegro, the former promising to donate a considerable sum of money if he won a lawsuit against the Jesuits, and the latter to bequeath part of his fortune upon his death. In 1748, the city council appointed Dr. Leonardo Sudrot de la Garda as city attorney, who presented the first approved plan for the founding of the charity hospital, donating his pharmacy and the necessary funds.

Due to incidents arising from the war between Spain and its neighboring countries, Dr. Sudrot encountered difficulties that prevented the execution of his plan. On September 9, 1751, he successfully acquired a house located at the current location of 4th Avenue between 9th and 10th Streets, the property of Don Nicolás Pérez Serrano. The deed was notarized at the Sole Notary Office before the scribe Don José Vernaza.

The house was renovated and inaugurated on January 23, 1753. The council allocated the "ninths of the tithe income" to contribute to its maintenance. Doña Francisca Ramos de Morales, Dr. Sudrot's wife, left a will in favor of the Hospital. She died in 1758. The Royal Spanish Court, at Dr. Sudrot's request, sent the brothers of the San Juan de Dios community to take charge of the Hospital in 1759. Fray Juan de Heredia and Fray Juan de Umaña arrived. In 1767, Dr. Sudrot died, having bequeathed all his assets to the Hospital. Fray Juan de Heredia was director until 1774.
