The Hospital in Veliko Tarnovo is the first hospital ever established in Bulgaria during the Ottoman rule. The initiative to create this hospital was led by Mihail Kefalov in 1854 with the goal of promoting health and education for the Bulgarian people. The hospital was founded through his will and executed by prominent local figures, including Dr. Vasil Hadzhiberon, Hadzhi Slavcho Hadzhipaskalev, Yanko Staneoglu, Hadzhi Hristaki Stoyanov, and Dimitar Stanchov. Over the years, the hospital grew, transformed, and contributed significantly to the medical and cultural landscape of Bulgaria.

The first hospital under the name "Saints Cosmas and Damian, the Unmercenaries" was opened in 1867 and was managed by Stefan Karagyozov with Yanaki Zlatev as the doctor. Despite facing financial difficulties, the hospital continued its activity in different forms and locations. Eventually, a first-class hospital with 60 beds was opened in 1879, and in 1888, it was relocated to a new, specially built facility, which is now Base II.
