The San Rafael Hospital of Itagüí began in 1951, when the most prominent members of the community met at the Municipal Hall and formed a Board to draft the bylaws for a local hospital, which were approved by the Ministry of Hygiene in 1952.
In 1953, Mrs. María Teresa Vélez Uribe donated some land to the municipality, and construction of the current hospital began. In 1966, the municipality donated an adjacent plot of land for the initial expansion, which was completed in 1972. That same year, its bylaws were amended, establishing the San Rafael Hospital of Itagüí as a non-profit organization dedicated to serving the community.
In 1979, the Hospital was attached to the National Health System; Later, in 1982, through a reform of the bylaws and by decision of the Board, its facilities and staff were handed over to the Antioquia Sectional Health Service.
Then, in 1988, it was included in the Itagüí Municipal Development Plan, becoming the central hub of the local health services network. Thus, the process of consolidating its position as a second-level hospital for healthcare and a referral site for health units in the southern Aburrá Valley began.
In 1994, to adapt to the new Social Security System, the Honorable Departmental Assembly transformed the San Rafael Hospital of Itagüí into a State Social Enterprise.
On February 15, 2007, the hospital took over operation of the former ISS headquarters in the municipality of Itagüí, the Santa María del Rosario Clinic. On December 19, 2007, the institution received an Honorable Mention in the competition held by the department of Antioquia, "Hospital Excellence Award 2007-2008," as a benchmark for successful experience in Strategic Management.
