About Us :

UPFH Family Clinic West Jordan is a non-profit, Federally Qualified Health Center located in West Jordan, Utah. Since opening in 2019, the clinic has expanded to serve a growing community with integrated medical, behavioral, and pharmacy services. The clinic is part of Utah Partners for Health, a network dedicated to providing affordable, accessible care to underserved populations. With a focus on compassion, inclusivity, and innovation, UPFH Family Clinic West Jordan strives to be a health home for families and individuals, regardless of insurance status or ability to pay

Speciality

UPFH Family Clinic West Jordan offers:

Primary medical care for all ages, including preventive and chronic disease management
Behavioral and mental health services, including counseling and substance use disorder treatment (MAT)
Pharmacy services and medication counseling
Dental and vision care (in collaboration with other UPFH locations)
Women’s health, pediatric care, and immunizations
HIV, STD, TB, and hepatitis testing and early intervention

From the moment I walked in the front desk ladies were very polite and courteous. Dr Tania and the nurse helping her were amazing. Dr Tania was very concerned and knowledgeable of what was going on with me. She went beyond! Thank you! Cost for me to use them and pharmacy was a total shock to me. I thought would be paying over $100 for everything nope not even the case. I love this place and I would recommend anyone and everyone to come here.

These people GENUINELY care about you. From Seno, Olympia, Jackie, to Dr. Tony and ALL the other doctors that are part of mat program are the absolute best. The health care I get here makes me feel like I'm in Canada or the UK. They even paid for my Lyft!!! EVERYONE employed here is great. I don't know how they did it.

It was a good experience from checking in at the front desk to filling the prescriptions after my visit! They seemed to have a good system for getting me checked in and ready to see the doctor. I didnt have a long wait for anything, not even for the prescriptions which were able to be filled conveniently right there (super nice)! I was treated with courtesy, empathy, and professionalism. I highly recommend Dr Xibille and the entire UPFH team! Thank you!
